     information needed on the Dues Statement. (We cannot guarantee security for
     internet transmission of your credit card.)

2.  Print the form, fill it out, then fax it to us at 315-471-0705.  If you fax it to us,
     please include your credit card number on the form.

3.  Print the form, fill it out, then mail it to us at:
